The ASTERIA project – Improving Therapeutic Monitoring In Real Life is an innovative project, supported by ARS Burgundy Franche-Comté and ERDF Burgundy, is jointly led by the CHU Dijon Bourgogne and the Burgundy Franche-Comté grades. The project aims to improve the care between the city and the hospital by setting up an organisation to monitor routine care (examinations and therapeutic follow-up) after leaving hospital. Its objectives: — Optimise patient follow-up in real time: anticipating non-compliance, detecting possible iatrogeny; — Improve long-term management and health pathway in chronic patients. The technological component aims to implement an urbanised remote monitoring platform in the ecosystem of the Burgundy Franche-Comté region. The platform must allow multiple prioritisation criteria to be crossed automatically and in real time: drugs, dosage, biology, history, lifestyle,... Digital technologies will facilitate accurate follow-up of treatments and regular checks defined according to patient characteristics as part of post-hospital follow-up.
