The aim of the project is to implement quality measures and organizational solutions in the health care system facilitating access to affordable, sustainable and high quality health services by developing and disseminating model diagnostic and therapeutic regimens of cardiac and oncological treatment.The target group, depending on the function performed in the project, will be primary healthcare facilities (POZ) and hospitals. The implementation of the project will contribute to increasing the efficiency of the public health system, especially by using the remote consultation platform for the ongoing exchange of medical recommendations between doctors of highly specialised medical centers and doctorsPOZ.The project will also contribute to the increase of the competence of the medical staff of the POZ in the management of cardiologically and oncologically ill patients. Within the framework of the project, there will be a selection of entities to whom grants will be awarded for the implementation of tasks.As part of the project, recommendations will be developed regarding the minimum scope of the model of diagnostic and therapeutic schemes in the field of cardiac and oncological treatment, and on their basis the selected lead centres will prepare a model of diagnostic and therapeutic scheme for a given specialisation. Next, the developed model will be verified on the basis of medical cases requiring further diagnosis selected by POZ institutions and partner hospitals, and the results of the verifications will be taken into account in the finalisation of the diagnostic and therapeutic scheme model.At the end of the project, an evaluation of the solutions will be carried out, and on its basis a report on the analysis of the possibility of implementing diagnostic and therapeutic schemes in other fields of medicine using the telemedicine platform will be carried out.
