The CHRU's objective is to create an e-ETP platform that takes into account the needs of patients and healthcare professionals as part of their journey. This platform will make it possible to carry out therapeutic patient education (TPE) in various ways: remotely or in a hybrid way and will contribute to the development of the continuity of the City-Hospital programs by being deployed throughout the territory. This project will provide patients, loved ones and professionals with access to FTE programs. In addition, the e-ETP platform will contribute to the continuity of programs between the hospital and the city by being deployed throughout the territory to develop a necessary transversal coordination between intra and extra hospital by placing the patient at the center of the device. The objective is to support, collaborate and transfer FTE skills to other actors in the territory, with the ultimate goal of improving patient care and strengthening the e-Health culture in our territory.
