Diabetes Mellitus Type 2 (DMT2) is a significant and growing health problem in the Netherlands. People with DMT2 experience barriers to access to care and self-management, such as limited access to healthcare facilities, lack of knowledge about diabetes management, and limited support for behavioral change. The Diabetes2CARE project tackles these barriers using an advanced digital application, the DiavantisWise(r). This application is scientifically based and integral, promising a shift in the approach to care for DMT2 from reactive to proactive. By means of personalized digital support, the DiavantisWise(r) enables patients to better manage their diabetes on a daily basis. The application focuses not only on the symptomatic control of the condition, but also on strengthening the self-management of patients. This allows patients to independently monitor their health, adjust their lifestyle and better deal with the challenges of DMT2.
