In the ChaRM project, a digital medication assistant is further developed and demonstrated in a relevant care environment, with the aim of improving medication management and adherence in primary and secondary care. This intelligent tool is linked to EHR and pharmacy systems and takes into account patient-specific skills. In addition to better patient experiences, digital medication verification contributes to more efficient use of scarce care capacity, with a potential saving of €0.5 million per hospital. The project is a collaboration between Zorgdoc Nederland, Elisabeth-TweeSteden Hospital and Tilburg University, and uses the joint expertise of these parties to create an impactful and user-friendly innovation. It supports policy objectives of Stimulus and RIS by promoting inclusive care and responsible medication use, while focusing on scalable implementation and greater impact within the healthcare market.
