MAIN OBJECTIVE OF THE PROJECT: Improving the quality of life of at least 150 inhabitants of the Commune of Rogowo (120 women, 30 men) by increasing the availability of integrated social services, responding to the diagnosed needs of the residents of the commune through the establishment, implementation and operation of the Social Services Centre in the Commune of Rogowo (CUS) in the period from 01.07.2024 to 30.06.2027. The TARGET GROUP consists of at least 150 inhabitants of the Commune of Rogowo, who belong to at least one of the following groups: -people in need of support in everyday functioning (including due to age, health condition, disability; OPW), -actual carers of people in need of support in everyday functioning (OF), -persons/families in crisis (O/RwK), i.e. people experiencing domestic violence (ODPD). The project implements the specific objective for measure 8.24, i.e. ESF+.CP4.K-Increasing equal and rapid access to good quality, sustainable and affordable services [...] through the establishment and operation of CUS Rogowo, which will provide social services to the residents of the Commune of Rogowo in accordance with the principle of CUS operation in accordance with Art. 1 of the CUS Act. The project will contribute to the achievement of the specific objective, also by improving the satisfaction of the needs of community residents of the municipality in contact with one institution (one-stop-shop model) and using the case management formula, which will ensure that the services offered respond to the real needs of specific residents, are provided in a timely manner and of appropriate quality. Residents will be provided with a catalog of services, the implementation of which will cause the desired change in their lives. The project is consistent with the type of project that can be implemented in the competition (indicated in the SZOP), implementing activities within the scope of the project type: 1-3,9,11-13 (C3). The concept of CUS operation, which is the subject of the project, is the result of the arrangements with ROPS in Toruń (C13). Project tasks: 1.Establishment of the Social Services Centre in the Commune of Rogowo 2.Implementation of social services within the Social Services Centre in the Commune of Rogowo L. persons covered by community-based services in the programme – 130 persons (104K,26M) L de facto/informal supervisors supported in the programme-20 pers.(16K,4M) Information about the project will be made available, among others, on the Applicant's website, in the tab on the activities of CUS Rogowo and by direct contact of CUS employees during information-prom meetings with potential recipients of services, the local community. The inform.-prom. campaign will be conducted through the distribution of posters/leaflets, in places characteristic for recipients of community services, i.e. clinics, commercial outlets, or through parish announcements. The project is in line with the Charter of Fundamental Rights of the European Union, implements the rights, freedoms and principles indicated therein, i.e. dignity, freedom, equality, solidarity, inter alia in the field of non-discrimination on the grounds of, inter alia, sex, race, colour, ethnic or social origin, genetic features, language, religion, etc. and the Convention on the Rights of Persons with Disabilities, as it seeks to protect the rights and dignity of persons with disabilities (OCD), promotes participation and integration in OZN society and inclusion in all aspects of society. The activities carried out in the project are voluntary and available to OzN, implemented as close as possible to the community in which they live, conducive to the development by OzN of their personalities, talents and creativity, as well as mental and physical abilities. Design shall ensure the availability and use of universally designed goods, services, equipment and devices as defined in Article 2 of this Convention.
