PURPOSE OF THE PROJECT: Increasing the availability and diversity of social services and improving the coordination of their provision through the introduction of three new services at the level of the City and Commune of Nakło nad Notecią (MiG Nakło) and the establishment of the Nakielskie Centre for Social Services (CUS Nakło) from 1.01.2025 to 31.12.2027 the project is addressed to a wide range of stakeholders and people using or who should be supported in the form of social services offered by CUS Nakło as a continuator of the Municipal Social Welfare Centre (MGOPS): 1) people in need of support in everyday functioning (including due to age, health condition and disability) and their surroundings 2) people caring for people in need of support in everyday functioning 3) people experiencing domestic violence Number of people covered by community-based services in the programme – 55 Number of actual/informal carers supported in the programme – 19 Number of people providing community-based services through support in the programme – 20 TASKS, i.e. the scope of the project is based on an analysis of the social services needs of the local community and will directly translate into the achievement of the objective and result indicators: 1 – Establishment and operation of the Social Services Centre: the transformation of MGOPS into CUS; enabling day-to-day operations for the first 33 months, creating the foundations and conditions for the implementation of CUS' statutory tasks: stimulating active citizenship, active participation in culture and health promotion; disseminating social services in the local community, ensuring the functioning of content teams within the CUS and the Centre for Support for Persons and Families in Crisis 2 – Care services: increasing the volume of people supported in the form of care services provided at home 3 – Respite care: increasing the availability of this form of support for actual carers by financing daily respite care at the place of residence and provided 24 hours a day 4 – Support for people forming part of the target group: setting up a support centre for people and families in crisis, where psychological and social counselling and legal consultation activities will be carried out – a comprehensive service consisting of these three areas will be outsourced to PES or NGO 5 – New services provided in the local community: implementation of three new social services on a municipal scale relating to various aspects and forms of care: Neighborly help, bond animator and mobile physiotherapist. The first two (implemented by PES or NGO) are primarily aimed at ensuring contact with the environment, taking care of mental condition and counteracting the feeling of loneliness. The third focuses on caring for physical condition, promoting a healthy lifestyle, physical activity and prevention in the field of maintaining physical fitness by seniors PLANNING INFORMATION ACTIVITIES: MGOPS/CUS has an insight into the needs of the charges living in MiG Nakło. Proven channels will be used to communicate with potential participants and other stakeholders: Internet (MGOPS/CUS website and the City and Municipal Office - UMG, local information services), posters (exhibition in places to which the representatives of the target group have access: MGOPS/CUS, UMG, municipal and parish bulletin boards), personal contact (meetings, transmission of information by employees in the field, organization of meetings at the City Council, in which representatives of parishes participate) www.naklo.pl and https://mgops.gmina-naklo.pl
