Daily Help House in the commune of Dąbrowa. Care services for residents of the district of Mogilno

The main objective of the project is the development of care services and specialist care services provided at the place of residence and in day-care centres - Day Care Home (hereinafter referred to as DDP), for 70 people in need of support in everyday life (hereinafter referred to as GIP), provided in the local community of the district of Mogilno in the period 01.01.2025 - 30.06.2027. This objective will be achieved by creating 10 places for the provision of care services in the day-care centre and organising 40 new places for the provision of non-residential care services (including 12 places for the provision of care services, 4 places for the provision of specialist services, 24 places for the provision of neighbourhood services). The project contributes to Objective 4. A more social, inclusive and implementing Europe’s Pillar of Social Rights, as set out in the European Funds for Kujawy and Pomerania Programme 2021-2027 and in SZOP FEKP.08.24 SOCIAL AND HEALTH SERVICES, and in particular the specific objective ESF+.CP4.K - Enhancing equal and timely access to quality, sustainable and affordable services, including services that support access to housing and person-centred care, including healthcare; modernising social protection systems, including promoting access to social protection, with a particular focus on children and disadvantaged groups; improving accessibility, including for persons with disabilities, effectiveness and resilience of health systems and long-term care services, as well as compliance in terms of area, forms of support, target group and output and result indicators. The project also falls within the scope of intervention 158 - Measures to increase equal and rapid access to quality, sustainable and affordable services. The project assumes the implementation of 8 substantive tasks: Zad 1. Setting up a Daily Help House for Zad 2. The day-to-day running of the Zad 3 Day Care Home. Activation support for the Daily Help House Zad 4. Services enhancing mobility autonomy and safety of people in need of support in the day-to-day functioning of Zad 5. Providing care services for people in need of support in everyday functioning in their place of residence Zad 6. Providing specialist care services for people in need of support in the day-to-day functioning of Zad 7. Provision of neighbourhood care services to people in need of support in day-to-day operations Key project indicators: The number of people covered by services provided in the local community in the program - 70 pers. The number of people providing services in the local community thanks to the support in the program - 24 pers. The number of created places of providing services in the local community - 10 pers. The project is implemented in partnership with the Municipality of Dąbrowa, in accordance with EU and national legislation, taking into account the principles of equal opportunities for women and men, equal opportunities and non-discrimination, including accessibility for persons with disabilities, in accordance with the Charter of Fundamental Rights of the European Union of 26 October 2012 and the Convention on the Rights of Persons with Disabilities, done at New York on 13 December 2006, and the principle of sustainable development.
