Project implemented by the Municipality of Aleksandrów Kujawski in partnership with 4PRO Grzegorz Grześkiewicz between 01.01.2025 and 31.12.2027. The aim of the project is to increase the access of residents of the Municipality of Aleksandrów Kujawski to integrated social services that meet the needs of the local community by transforming the Municipal Social Assistance Centre into a Social Services Centre and implementing social services until 31.12.2027. The project is addressed to the residents of the Municipality who meet the requirements specified for the target group. The project assumes the main objective, which is the creation of the CUS and individual specific objectives related to the functioning of the newly established institution: 1.Increasing the availability of care services for at least 30 people as part of the operation of the Daily Help House in Słomków by creating 5 additional places. 2. Increasing mobility and autonomy for at least 30 people in need of support in everyday functioning from the Municipality of Aleksandrów Kujawski by launching a door-to-door service. 3. Increasing security and access to support services for at least 20 people by creating a Crisis Intervention Point in the Aleksandrów Kujawski Commune. 4. Increasing the availability of specialist care services in the place of residence for 15 people requiring assistance in the form of specialist services, resulting from the type of their disease or disability from the Municipality of Aleksandrów Kujawski. 5. Minimizing the negative effects of disability and preventing the deterioration of the health condition of 20 residents of the Commune by providing rehabilitation and medical equipment. Main outputs and results to be achieved through the implementation of the project: - Number of service places created in the local community - 5 - Number of people covered by services provided in the local community in the programme - 90 Main tasks of the project: 1) Establishment and operation of the Social Services Center 2) Daily Help House 3) Door to door service 4) Emergency intervention service 5) Specialist care services in the place of residence 5) Rental of equipment necessary for care and rehabilitation Planned information activities, indicating the address of the website on which the project description will be found. Information and promotion activities and information about the possibility of participating in the project will be carried out by the Municipality and the Partner. The lead partner will distribute posters and leaflets inform., which will be available in the Commune Office and organizational units of the commune, at stops, in health centers, shops, parishes. In addition, the Municipality will provide direct contact with information about the project, which will be communicated to the mayors, social workers, local NGOs, parish priests, via social media. The municipality will organise at least 2 information meetings in 2025 and 2026 on the forms of support in the project. The partner will develop models of promotional and recruitment documentation. Information about the project and the opportunity to participate will be available in all places of support implementation. The support offered in the project is tailored to the individual needs, potential and personal preferences of the recipients of these services (especially in the case of people in need of support in everyday functioning and people with disabilities). In addition, the support is tailored for people excluded from communication.
