The project is implemented by the Municipality of Luban in partnership with the Polish Economic Society – Bydgoszcz Branch and Zofia Pachnik between 01.01.2025 and 31.12.2027. The aim of the project is to increase the access of the inhabitants of the Municipality of Luban to integrated social services that meet the needs of the local community by transforming the functioning of the Municipal Social Assistance Centre in Luban into a Social Services Centre by 31.12.2027. The project is addressed to the inhabitants of the Municipality of Luban who meet the requirements set out for the target group (described in the field below). The project assumes the main objective, which is the creation of CUS and individual tasks/specific objectives related to the functioning of the newly established institution: 1.Increasing mobility, autonomy and safety of people in need of support in everyday functioning by providing door-to-door transport for at least 60 people in need of support in everyday functioning (42 people not duplicating support from other services). 2.Increase access to care services for 20 people in need of daily support by creating 20 places in the Daily Care Home. 3. Enhancing opportunities for residents to meet their living needs through the introduction of neighbourhood services. 4.Reducing the effects of disability and preventing the deterioration of the health condition of 20 residents of the Commune through the service of renting rehabilitation equipment. 5.Counteracting domestic violence and increasing the availability of support for 30 people experiencing crisis through the creation of a Crisis Intervention Point providing short-term shelter and specialist psychological, pedagogical, social and legal support. Main outputs and results to be achieved through the implementation of the project: Number of created places of providing services in the local community - 25 Number of people covered by services provided in the local community in the program - 132 Number of people providing services in the local community thanks to the support in the program - 20 Information and promotion activities and information about the possibility of participation in the project will be implemented by the Municipality, PTE and Zofia Pachnik. The lead partner will distribute posters and information leaflets, which will be available in UG Luban and organizational units of the municipality, at stops, in health centers, shops, parishes. In addition, the Municipality will provide direct contact with information about the project, which will be communicated to the mayors, social workers, local NGOs, parish priests, via social media. Zofia Pachnik will organize at least 2 information meetings 2025 and 2026 on forms of support in the project. The PTE Partner will develop models of promotional and recruitment documentation. Information about the project and its description will be available at: lubanie.com Sustainability of the project after completion of its implementation will be ensured at least for the period corresponding to the period of implementation of the project and sustainability in accordance with the Guidelines on eligibility of expenditure. The project will be implemented in accordance with the provisions of local and national law, guidelines, competition rules and standards indicated in the annexes to the rules. The applicant shall ensure that all services are provided in accordance with accessibility standards. At the recruitment stage, people from the target group will be preferred: a) severe/moderate handicaps, b) coupled handicaps, c) mental disorders, d) intellectual handicaps, e) holistic developmental disorders (as defined by the International Classification of Diseases and Health Problems ICD10), f) F.E.P. beneficiaries, g) living alone, h) in a homelessness crisis, affected by housing exclusion/at risk of homelessness (in terms of housing support), i) children raised outside the biological family.
