We support, activate and take care of!

The project aims to increase the availability of special care services provided in the place of residence and in day support centres for a total of 290 people and to include 165 actual carers residing in the City of Bydgoszcz and the municipality of Sicienko in the period from 6.2024 to 10.2027. Created 2 new outlets - Daily Help Houses, 1 outlet will be retrofitted and expanded. Part-time care will also be provided in the project area. MAIN PRODUCTS: - Establishment, adaptation and equipment of 2 new facilities - DDP (in the City of Bydgoszcz and increase of places and further operation of 1 day care facility - Daily Help House in Mochel) (Total: 60 new day care places (DDP): 25+30 in Bydgoszcz and 5 in Mochel) - 225 people in need of support in everyday functioning (support in DDP) - 65 people in need of support in everyday functioning (support in the place of residence) - 165 actual carers (increasing knowledge about caring for a dependent person) A. STATIONAL SERVICES Participants in DDP (225 people) will be only people in need of support in everyday functioning (as defined by the competition regulations). Preferences for people in a particularly difficult situation, i.e. Persons with disabilities and in need of support in everyday life, suffering from dementia and those whose income does not exceed 150% of the relevant income criteria (according to the Social Assistance Act of 12 March 2004). On-site support in the form of specialist workshops and advice ind. covered by 165 people. (actual carers - caring for people in need of support on a daily basis ). Support for people with DEMENCIAL DISEASE will cover at least 10 pers. (Total 10 places created in 3 DDP) B. NON-STARNATIONAL CARE SERVICES Covered by: 65 people in need of support in everyday functioning in the place of residence (target group according to the rules of the competition). Preferred during recruitment will be people in a particularly difficult situation, i.e. Persons with disabilities and in need of support in everyday functioning, whose income does not exceed 150% of the appropriate income criteria (per person living alone or per person in the family), referred to in the Act of 12 March 2004 on social assistance. MAIN TASKS: Zad.1.Repair and equipment: DDP CHOPINA Functionality: DDP CHOPINA Zad.3.Renovation and equipment: DDP OF DEFENCE Task.4.Functioning: DDP WARNINGS Zad.5.Permanent care services (BYDGOSZCZ) Zad.6.Fitting out: DDP IN MOCHLU Zad.7.Functioning: DDP IN MOCHLU Zad.8.Permanent care services (MOCHLE) Zad.9.Intermediate costs Total: 290 (220K/70M) + 165 people (120K/45M) caregivers fact. Within the deadline: 10.2024-01.2025 -Recruitment (participants and staff), selection of contractors, renovation and adaptations 02.2025-10.2027 - In-patient and residential care + support of DDP's actual guardians will operate all year round on working days min. 8h/day and will meet the standards of Day Help Houses (Appendix No. 10 to the Competition Call). Participants will be provided with additional therapeutic activities in the form of meetings with a dietitian (lectures, culinary training,) specialist in the field of nursing and health prevention, physiotherapist, psychologist, occupational therapist (thematics adapted to the needs and interests of the target group. The activities will comply with the Minimum requirements for the provision of social services in the local community (Annex 9 to the Regulations for the selection of projects). INFORMATION ACTIVITIES - Description of the project on the Beneficiary's website and on www.cus.byd.pl in the Daily Help Houses tab and on the Applicant's social networks. During the promotional activities, equality between women and men will be promoted by building information and promotional messages based on messages free from stereotypes (language, graphics, images). Information activities will be carried out in accordance with the rules of information and promotion applicable in the Programme.
