Day Support Centre - Common Case

The project assumes the creation of a Daily Relief House in Piechcin, Barcin commune, which will offer support for 15 people in need of support in the daily functioning of the Barcin commune living (in accordance with the Central Committee) and the implementation of care services for this group along with the creation of places for people with dementia. The project will be implemented by the Local Action Group Pałuki-Joint Case in partnership with the Municipality of Barcin. The aim of the project is to increase access to social services for 15 places for people in need of support in the daily functioning of residents (according to the Institute of Social Sciences and Humanities). KC) in the Municipality of Barcin (Kujawsko-Pomorskie Voivodeship) until 30.06.2027. DDP's activity will be based on providing a safe place to stay daily and support in self-service activities, as well as care for people who cannot function without the help of other people. The implementation of the project will start with adaptation and renovation works and the purchase of equipment and equipment in order to adapt the premises to the implementation of the services provided under the project. This work will include, among others, adaptation and renovation works as well as the purchase of equipment and equipment. For this purpose, an adaptation of approx. 150 m2, which will ensure the possibility of creating and proper functioning (providing comfort and safety) of 15 places. The DDP will operate throughout the year, on all working days, at least 8 hours a day, in hours adapted to the needs of students and families, and will have 15 support places, including at least 6 created places for people with dementia, along with the provision of an offer of support tailored to the needs of this group, including tailored forms of therapy and impact of an individual and group nature. The total duration of the participants' stay in the Daily Help House will be determined on the basis of the needs of the participants and their families and will, as a rule, assume constant support. The participants of the project will be provided with full-day care of 5 carers (3 posts planned to provide appropriate care and support for people with dementia), meals consisting of a minimum of two meals a day (including lunch) adapted to the individual nutritional needs of the participants, transport to and from the center, in particular for people who have difficulty in moving in an adapted means of transport - a bus adapted to the requirements of people with disabilities, as well as access to specialist services, e.g. psychologist, physiotherapist, rehabilitator depending on the needs of the participants. The project assumes partial payment for services provided under the project adjusted to the financial capabilities of the participants, with the exception of persons not exceeding the income criterion indicated in the Act of 12 March 2004 on social assistance. Amount of 150 PLN/month. The main output of the project will be the number of people covered by services provided in the local community in the programme – at least 36, and the main result will be the number of service places created in the local community – 15. The DDP project will be implemented in accordance with the Standard of Day Help Houses constituting Annex 10 to the Project Selection Regulations. The applicant undertakes to maintain the sustainability of the project in accordance with Article 65 of the General Regulation. Duration of the project: 01.07.2024 - 30.06.2027. All information and promotion activities, documentation and purchased equipment will be marked in accordance with the "Manual of the applicant and beneficiary of European Funds for 2021-2027 in the field of information and promotion".
