Project implemented by the Municipality of ALEKSANDRÓW KUJAWSKI (JST) in partnership with the SOCIAL INITIATIVES FOUNDATION ŹRÓDŁO (PES) and 4PRO Grzegorz Grześkiewicz. The entity participating in the implementation of tasks (incurring expenses) from 01.01.2025 will be the Social Services Centre in the Municipality of Aleksandrów Kujawski. Project implementation period from 2024-07-01 to 2027-06-30. The aim of the project is to increase the availability of care services for 80 people in need of support in everyday functioning as part of the operation of two Day Assistance Houses in Słomków and Wołuszew within the newly created 25 places (including 6 for people with dementia diseases). The objective will be achieved by 30.06.2027. The project is aimed at people in need of support in everyday functioning from the rural commune of Aleksandrów Kujawski. The main indicators to be achieved through the implementation of the project are: Output indicators: - Number of people covered by community-based services in the programme - 80 Result indicators: - Number of establishments in the local community - 25 Other common output indicators: 27 The project assumes a higher number of participants than the number of places where care services are provided, which is due to objective factors (e.g. resignations due to deterioration in health, family reasons, change of residence, death). This assumption was made on the basis of previous experience in the provision of care services. However, the project does not assume rotation of participants as part of periodic calls (turnus). Information and promotion activities and information about the possibility of participation in the project will be implemented by the applicant and Partners in the implementation area. The lead partner will distribute posters and information leaflets, which will be available in the Municipal Office and subordinate organizational units, at stops, health centers, shops, parishes. In addition, the Applicant and Partners will ensure direct contact with information about the project, which will be communicated to the mayors, social workers, local NGOs, parish priests and via social media. The applicant will organise at least 2 information meetings on the forms of support in the project. Information about the project and its offer will be available on the websites of the applicant and partners (https://www.facebook.com/ddp.słomkowo and https://gmina-aleksandrowkujawski.pl/). The project will be implemented in accordance with the competition rules and standards indicated in the annexes to the rules and national law. The service will be implemented in accordance with the accessibility standards annexed to the Guidelines for the implementation of equality principles under EU funds for 2021-2027. Partial payment of participants for DDP services is foreseen. When awarding contracts, preferences for Social Economy Entities will be applied in accordance with the rules of the competition and social clauses. The applicant shall ensure that: - the project office located in the DDP and all activities will be implemented in buildings with architectural accessibility for people with disabilities, - digital materials (documents, promotional and information materials) will meet the accessibility criteria (according to WCAG 2.1 standards), - a form for reporting special needs of project participants will be prepared.
