Exclusion - there is no MOU! 2 - Stage I

As part of the project, an integrated system of activities will be created to support children and young people currently residing in educational, rehabilitation and sociotherapeutic institutions in the Kujawsko-Pomorskie Voivodeship. The project will aim to improve access to support services for children and young people who have problems with proper functioning in society. This will enable minors to function better and in accordance with standards in the future. In addition, the project will focus on the deinstitutionalization of rehabilitation and sociotherapeutic processes by conducting activities aimed at empowering pupils and enabling them to function properly outside the closed institution. These actions will contribute to increasing equal and timely access to quality, sustainable and affordable services. In accordance with the recommendations included in the Regional Plan for the Development of Social Services and Deinstitutionalisation for the Kujawsko-Pomorskie Voivodeship for the years 2023-2024, the project will focus on the development of services enriching the rehabilitation process, preparing for further social roles in adult life for people staying in 24-hour institutions, including MOW, MOS and support for the process of emancipation of children and young people. Project activities will also focus on the prevention of mental health of young people by enabling access to social services in the form of psychiatric and psychological consultations, therapy, support groups and by organizing seminars in the above-mentioned area. The project, through the creation of training apartments, will support young people in the process of independence, while counteracting the phenomenon of homelessness of young people. In addition, in order to respond to the needs of the target groups, it is planned, as part of the support for children and young people, to enable specialist staff working with young people, including MOW, MOS staff, probation officers, to participate in training in order to prepare specialists providing social services for children and young people preparing to leave institutional forms and being ‘at the border’ of placement in an institution. In addition, it is planned to organise awareness conferences as part of the project, especially in the area of prevention of mental health of young people. All services offered by the project will be implemented in accordance with the definition of community-based services set out in the Guidelines for the implementation of European Social Fund Plus projects in the 2021-2027 regional programmes. As part of the project, the following tasks will be carried out: 1. Prevention of mental health disorders of children and adolescents in closed institutions. 2. Classes increasing the competences of young people staying in closed institutions in terms of independence and proper functioning in society. 3. Other forms of educational, rehabilitation and sociotherapeutic work. 4. Support in the self-reliance of people leaving 24-hour facilities. 5. Support for staff providing services to children and young people.
