MAIN OBJECTIVE OF THE PROJECT: Improving the quality of life among at least 175 inhabitants of the Commune of Strzyżewice (110 women/hereinafter: K, 65 men/hereinafter: M) by increasing the availability of integrated social services responding to the diagnosed needs of the commune residents through the establishment, implementation and operation of the Social Services Centre in the Commune of Strzyżewice (hereinafter: CUS) in the period 01.01.2025-31.12.2027. DOCUMENT GROUP consists of at least 175 inhabitants of the Commune of Strzyżewice (110K,65M), who belong to at least one of the following groups: -persons at risk of being placed in 24-hour institutions or persons with disabilities; -actual guardians; People at risk of social exclusion. The project is addressed only to people who are studying, working or residing in the Lubelskie Voivodship within the meaning of the provisions of the Civil Code. Activities related to the establishment of CUS and support for services provided by CUS will be carried out in accordance with the Act of 19 July 2019 on the implementation of social services by the Social Services Centre (hereinafter: The CUS Act). The implementation of social services under the project will be carried out in accordance with the principles and standards indicated in Annex 12 to the Regulations for the selection of projects: Detailed rules for the implementation of support for the establishment or operation of social services in the local community and their services, including CUS. The project implements the specific objective 4k - Increasing equal and rapid access to good quality, sustainable and affordable services [...] through the establishment and operation of CUS Strzyżewice, which will provide social services to the residents of the Commune of Strzyżewice (hereinafter: GS) in accordance with the principle of operation of the CUS in accordance with Art. 1 of the CUS Act. The project will contribute to the achievement of the specific objective, also by improving the satisfaction of the needs of the community of GS residents in contact with one institution (one-stop-shop model) and using the case management formula, which will ensure that the services offered respond to the real needs of specific residents, are provided in a timely manner and of appropriate quality. Residents will be provided with a catalogue of social services compatible with the ESF+ intervention, the implementation of which will bring about the desired change in their lives. The information and promotion activities of the project will be implemented in accordance with the ‘Guidelines on information and promotion of European Funds for 2021-2027’, the ‘Guidelines on the implementation of equality principles under EU funds for 2021-2027’ and the Accessibility Standards for Cohesion Policy for 2021-2027. The information and promotion campaign, using various information channels, will be addressed in particular to the residents of GS, including groups of people constituting the target group of the project. Project tasks: 1. Milestone 1 (6 months)-Preparation and establishment of the CUS; 2. Milestone 2 (12 months) – Implementation of the CUS Implementation Plan and the provision of social services at CUS; 3. Milestone 3 (10 months) – Overview of services provided at CUS, update and implementation of the CUS Implementation Plan; 4. Milestone 4 (8 months)-Implementation of CUS services and monitoring of performance of indicators.
