The aim of the project is to increase access to social services, i.e. family support services for 150 children and adolescents (69 K and 81M) aged up to 18 years from the area of the Municipality of Skała, at least 105 (84K and 21M) of their parents/guardians and families of the above-mentioned children and adolescents by creating 30 places of care and upbringing within the PWD in the care-specialist and backyard function. Target group: children and adolescents under the age of 18 years of their family and a new day support facility was established. Main tasks of the project: Task 1 Creation and functioning of the PWD in the Rock Commune. Task 2 Diagnosis of needs and individual support plan and cooperation. Task 3 Care support. Task 4 Specialist support for the child and his family. Task 5 Backyard support. Main results: creation of 30 places of care and upbringing in the new PWD in Skala. Launching support in the form of assistance services in the care and upbringing of a child in the Skała Commune.
