Tarnów i powiecie tarnowskim w woj. małopolskim poprzez objęcie 30 osób potrzebujących wsparcia w codziennym funkcjonowaniu (22K,8M) usługami opiekuńczymi realizowanymi w nowo utworzonym Dziennym domu pomocy i sąsiedzkimi usługami opiekuńczymi, a także poprzez objęcie 30 opiekunów faktycznych ww osób (24K+6M) działaniami zwiększającymi ich umiejętności i kompetencje opiekuńcze prowadzonymi w Centrum wsparcia opiekunów faktycznych i realizację działań informacyjno-edukacyjnych kierowanych do kadr związanych z opieką nad osobami potrzebującymi wsparcia w codziennym funkcjonowaniu w okresie 01.06.2025 r.- 30.09.2026 r. Cel projektu realizuje Cel polityki 4. A more social, inclusive and implementing Europe’s Pillar of Social Rights and Specific Objective 4(k) – “Strengthening equal and timely access to quality, (...)”: implementation of support for GIPs and OF will improve access to social services for these groups in Tarnów and Tarnów County and will contribute to reducing social problems, especially among the elderly, who, due to their difficult financial situation, would not be able to use paid services The target group of the project is 30 people in need of support in everyday functioning (22K, 8M) and 30 actual carers (24K, 6M) living in the City of Tarnów or Tarnów County in Małopolskie Province. The project will also implement information and education activities addressed to staff related to the care of people in need of support in everyday functioning (representatives of health services/social assistance/ district family assistance centers/PFRON/NGO), learners/workers/residing in the City of Tarnów or in the Tarnów district in the Małopolska province (it is assumed to include, among others, platforms for meetings and cooperation of at least 8 representatives of the above-mentioned staff) 1. Individualization of support in the Daily Help House 2. Creation of the Daily Help House and its basic activities 3. Daily house of help-therapeutic activities, in the field of life and physical activity and information and educational support 4. Daily help home - culturally and socially activating support 5. Neighborhood care services 6. Support center for real (informal) carers - basic activities, educational and advisory support for carers 7. Helpdesk for real (informal) carers - information support for carers 8. Support centre for actual (informal) carers - information and education activities addressed to care-related staff 9. Support center for actual (informal) caregivers - creation of rehabilitation, care and support equipment rental, respite care 1.
