The main objective of the project is to enable the return to the labour market of 50 UP (30K, 20M) and to maintain employment of 200 UP (120K, 80M) living in the Podlaskie voivodship until 31.12.2026, by providing this person with comprehensive support in the field of activating the labour market and rehab, individually tailored to the needs of each UP. The project will be implemented in the period from 02.01.2025 to 31.12.2026. The main objective of the project will be achieved through the implementation of the following tasks, planned so as to provide each UP with support in the field of activation of employment and rehab, tailored to individual needs, so as to facilitate the return to work / retention of employment: Rehab. med. Zad.1. Diagnosis, manual therapy and physiotherapy; Task 2. Specialty therapy. (urogynecological physiotherapy and Cranio-sacralna therapy); Zad.3. Group preventive workshops in the field of physiotherapy; and in terms of professional activation: Zad. 4. Psychological support; Zad. 5. Career coach support; Zad. 6. Vocational training to improve/acquire new qualifications and competences; Zad. 7. Traineeships. It is assumed that each of the UP will cover at least 1 task in the field of activation of the project and at least 1 task in the field of rehab, whereby the scope of the necessary support for a given person will be determined (in the field of activation of the project) on the basis of the application documents for the project, the employment situation of a given person and (in the field of rehab. and med.) on the basis of the subsection carried out in the Zad. 1 diagnosis of the medical situation and medical documentation by a qualified physiotherapist. A common path of actions will be determined individually for a given person thanks to close cooperation between L. and P., so that the actions provided to UP under the project will ensure the restoration of full or maximum physical and mental fitness facilitating return to work or maintaining employment and maintaining the person as long as possible on the labour market. The rehab support implemented under the project will be consistent with the data contained in the map of health needs for the Podlaskie Voivodeship. The project is in line with the document: A healthy future. The Strategic Framework for the Development of the Health System 2021-2027 with a Perspective to 2030" i.e. responds to Objective 1.1. (Accessibility) Ensuring equal access to health services in a quantity and time commensurate with the legitimate health needs of society and 1.4. (Public health) Development of prevention, effective health promotion and health-promoting attitudes of health threats. The project is consistent with the Podlaskie Voivodship Transformation Plan for 2022-2026, as it assumes improving the health condition of the inhabitants of the province, including a large group of people aged 50 and over. The target project will consist of 50 UP (30 K, 20 M) – persons registered as unemployed in the PLO in the Podlaskie Voivodeship and 200 Participants (120K, 80M) – persons employed in the Podlaskie Voivodeship within the meaning of the Civil Code who are exposed to leaving the labour market due to health factors, including at least 130 UP (80K, 50M) with the above-mentioned persons aged 50 and over. The main results of the project are: 250 UP subject to activation and rehab med. including: 250 UPs who will benefit from psychological support or career coach support, 35 UPs who will take part in vocational courses/training, including 28 UPs who will complete vocational training/courses and improve/acquire vocational qualifications/competences and 15 UPs who will complete 3-month vocational internships and 250 UPs covered by medical diagnosis, manual therapy and physical therapy, special-purpose urogynecological and/or cranio-sacral therapies and will benefit from health prevention workshops; 150 UP (90K,60M) who, thanks to support in the area of health, took up work or continued employment. The rehab services will be provided in accordance with the provisions issued on the basis of the Act of 27 August 2004 on health care services financed from public funds, in particular the Regulation of the Minister of Health of 6 November 2013.
