Main objective of the project: Increasing to 30.06.2027 the chances of maintaining employment and improving the health status of 720 people (432K, 288M) working in Białystok and Bialystok County (Podlaskie Province) within the meaning of the Civil Code regulations, exposed to leaving the labour market due to health factors through comprehensive medical rehabilitation and professional activation. The project will achieve the following results: • The number of people who, thanks to support in the area of health, took up work or continued employment - 612 The target group of the project will be 720 people (432K, 288M) working in Białystok or Bialystok County (Podlaskie Voivodeship), at risk of exclusion from the labour market due to health problems. The target group is identical to the one mentioned in the CPR. As part of the project, all participants will be covered by comprehensive support in the field of medical rehabilitation and professional activation - project implementation outside the formula of the RPZ (Special Criterion No. 2). The program of medical rehabilitation and professional activation will be tailored to the individual needs of each project participant (UP). The project includes support for workers to return to work after long-term sick leave and those returning to work after long-term incapacity for work and measures to prevent long-term incapacity for work. The project plans to carry out the following tasks: • Individual program of medical rehabilitation • Individual psychological support • Psychological support workshops • Workshops for professional development and activation The activities planned in the project are a response to the identified needs and problems of the target group resulting from DIAGNOSIS carried out by the Applicant in the period 11.2023-01.2024, i.e. before submitting the application for funding. project. based on available, verifiable data available for the area of support, i.e. the county of m.B-stok and b-stocki and based on a survey (questionnaire questionnaire) carried out on a group of 63 (41K, 22M) patients of CM Vittamed, benefiting from rehabilitation services. The conclusions of the diagnosis are presented in the description of the target group and the point entitled: ‘Diagnosed problem(s)’. Each stage of the project will be carried out in accordance with the principle of equal opportunities and non-discrimination, including non-discretionary disabled persons (DPOs), as well as equal opportunities for women and men (KiM) in accordance with the Guidelines on the implementation of equality principles under EU funds for 2021-2027. All tasks in the project have been planned in such a way that there are no equality barriers at any stage of its implementation. Schedules of meetings and workshops will respect the principle of equality between women and men, so that the hours of implementation of support allow for reconciliation of professional and private life of participants of the project. Implementation of the project's assumptions will be based on the Project Selection Rules (PRC) developed on the basis of the competition's assumptions. The project is in line with the objectives defined in the Healthy Future. Strategic Framework for the Development of the Health System 2021-2027 with a Perspective to 2030’, i.e. - Objective 1.1 [Accessibility] Ensuring equal access to health services in sufficient quantity and time to meet the justified health needs of society – the project will enable UP access to high-quality medical rehabilitation and professional activation, which will contribute to improving their health status, equal opportunities in working life and improving the quality of life. - Objective 1.4 [Public Health] Development of prevention, effective promotion of health and health-promoting attitudes of health threats – the activities planned in the project support health-promoting attitudes and have contributed to the prevention of health complications. The project is in line with the Voivodship Plan for the Transformation of the War. Podl. for the years 2022 - 2026, as the activities planned in the project are aimed at increasing the availability of high-quality rehabilitation services, which will improve the health status of the inhabitants of Podl.
