Home telecare programme – support programme for senior policy and persons with disabilities

The project is in line with the specific objective set out in the FEdP 2021-2027 ‘k’, i.e.: Increasing equal and rapid access to good quality, sustainable and affordable services (...) The project will support the following services: Neighbours, carers, specialist carers, assistants - provided in the place of residence, i.e. provided in the local community and telecare services, provided that the comprehensiveness of services is guaranteed. Additional support concerns the implementation of trainings for people performing neighborly services 8 hours and care 80 hours. The implementation of the info-promo activity is planned, delivery of small preventive equipment Care services will be provided for people in need of support in everyday functioning, and assistance services in particular for people with disabilities. The aim of the project is to support people in need of support in everyday functioning, including the elderly, with disabilities, with mental disorders, thus the services implemented under the project will be tailored to the needs of participants while maintaining equality in access. The project complies with the principles referred to in Article 9 of the General Regulation: the principle of equality between men and women, the principle of sustainable development, including the principle of ‘do no significant harm’ (DNSH), the principle of equal opportunities and non-discrimination, including accessibility for persons with disabilities, the Charter of Fundamental Rights of the European Union, the Convention on the Rights of Persons with Disabilities. The services will be provided at a time tailored to the individual needs of the participants, including in the morning, afternoon, evening, night, public holidays and public holidays, if the person uses such services and such a need is determined during the qualification. Neighbouring services include: assistance in meeting basic living needs, i.e. hygienic and nursing care, which should be understood as forms of support that do not require specialist knowledge and competence, and, where necessary and possible, ensuring contact with the environment. Care services include: assistance in meeting daily needs, hygienic care, care recommended by the drug, which includes activities resulting from the drug certificate or medical documentation, complementary to community nursing care, ensuring contact with the environment Specialist care services include services tailored to the specific needs resulting from the type of disease or disability and are performed by people with special professional preparation. The scope of services includes: care as support for the treatment process, physical rehabilitation and improvement of disturbed bodily functions in the scope not covered by the provisions of the Act on health care services Assistant services will include supporting persons with disabilities in performing basic everyday activities necessary for active social, professional and educational functioning in the place of residence. Depending on the needs of the person with incompleteness, the assistance service may also include hygienic care and assistance in physiological activities. Telecare services will be available24 hours a day, 7 days a week, wristband and connection to the telecare center The organizer of services will be the municipality (social assistance center) competent for the place of residence of the person using the services. The organization of the provision of services will take into account the subjectivity of the recipients of services, including respect for the right to respect and protection of dignity, intimacy, in particular in the case of hygienic and care activities and a sense of security and protection of personal rights. The support provided under the project will be tailored to the individual needs, potential and personal preferences of the participants, taking into account the diagnosis of a family situation, problem or threat of a problem situation, resources, potential, predisposition, needs based on an interview conducted before the participant's qualification.
