The main goal of the project is to increase access to integrated social and health services through the creation of a new Social Services Centre in the Krasnopol Commune (CUS) in the Podlaskie Voivodeship, which will develop and implement solutions ensuring universal access to social and health services for 310/ 65%K residents of the Krasnopol Commune (GK) who need to be covered by social and health services under CUS in the period from 01.04.25 to 31.03.2028. The TARGET GROUP of the project is 310 people/65%K who live in reason. Civil Code, they study or work in the area of the Commune of Krasnopol in the Podlaskie Voivodeship, in the Sejny County. These are people in need of social and health services, i.e. families (45 families / 180 persons), including dysfunctional families with care and educational problems; elderly people of post-production age, people with disabilities, people with special needs resulting from the type of disease or disability (130os./ 100K,30M). As part of the project, the following TASKS will be implemented: 1. Preparation for the establishment of CUS in Krasnopol - CUS 2 Implementation Plan. Implementation of the CUS 3 Implementation Plan. Review and update of the CUS Implementation Plan and implementation of the Plan after the update As a result of the implementation of the above-mentioned tasks, the following RESULTS and PRODUCTS of the project will be achieved: -40 community service places will be created (Senior Club) -8 family support and foster care places will be created after the end of the project -130 people will be covered by the services provided in the local community in the program -180 people will be covered in the area of family support and foster care -45 people with disabilities will be supported in the program Project through the implementation of activities related to the creation of a new organizational unit in the Municipality of Krasnopol - CUS in accordance with the Act of 19 July 2019 on the implementation of social services by the social services center (hereinafter the Act on CUS) contributes directly to the achievement of the objectives contained in the FEdP 2021-2027 program, in particular. ‘k’: Enhancing equal and timely access to quality, sustainable and affordable services, including services that support access to housing and person-centred care, including healthcare; modernising social protection systems, including promoting access to social protection, with a particular focus on children and disadvantaged groups; improving accessibility, including for persons with disabilities, effectiveness and resilience of health systems and long-term care services. CUS in Krasnopol will be created by transforming the social welfare centre functioning in the commune into CUS. CUS will provide and coordinate support in the area of social and health services, which is in line with the ‘Strategy for the Development of Social Services, Public Policy 2030 (with a 2035 perspective)’ and the Podlaskie Regional Plan for the Development of Social Services and Deinstitutionalisation 2023-2025, as well as the ‘National Programme for Combating Poverty and Social Exclusion’. CUS is one of the activities and tools in the process of deinstitutionalization leading to the development of social services in the environment, i.e. it implements the most important assumptions set out in the above-mentioned documents. The services will be implemented in accordance with the ‘Implementation of projects involving ESF Plus funds in regional programmes for 2021-2027’. Support from social services concerns only services provided in the local community. The project assumes the implementation from the project funds of a minimum of 4 types of social services from the scope of ESF+ interventions, which were not implemented in the area of CUS action, i.e. Commune of Krasnopol in the period of 12 months preceding the announcement of the call for applications for co-financing.
