Measures to increase the competence of care coordinators in POZ facilities

The aim of the project is to increase the competences of at least 2,100 care coordinators in order to support the implementation of coordinated care in the POZ, improve the quality and service of patients, improve the quality and effectiveness of the services provided in the POZ, over the period of 3 years of the project implementation. The project will have the task of unifying the competences of the coordinators and selecting a group of leaders (trainers) who will support this professional group after the end of the project. The Care Coordinator is a new profession in healthcare and is closely related to coordinated care. Coordinated care in the healthcare system consists in integrating the provision of healthcare services, covering all stages and elements of the process of their implementation, with particular emphasis on the quality and efficiency of the services provided. It is also a way of providing medical services that ensures continuity and comprehensiveness of diagnosis and treatment in chronic disease. Coordinated care in the part concerning the tasks of the care coordinator has been implemented in Poland since 1 October 2021. 2 of the Act on the pos., each POZ institution is obliged to appoint a person - a care coordinator. Bearing in mind that this is a new solution, a series of trainings should be carried out in which care coordinators will be able to improve their competences related to the tasks assigned to them. Any person who acts as a coordinator in a medical entity with a contract with the NFZ for coordinated care can take part in the trainings. The order of applications will apply, and the number of coordinators who can apply for training from a given unit will depend on its size, i.e. the number of patients treated by a given entity. The individual training plan will be determined on the basis of a questionnaire assessing the level of competence necessary for this position, completed by each candidate. Recruitment for care coordination leaders (trainers) will take place in two ways: 1. recruitment on general principles after completing the questionnaire of competence of the coordinator and leader (trainer). After reaching the established level of assessment, the candidate will be offered training as a leader (trainer); 2. coordinators who have completed basic training with a high score on the final test and who have a coaching predisposition (a competency survey of the coordinator of the leader (the trainer) will be offered participation in the training as a leader (the trainer). Training for coordinators will consist in filling competence gaps in defined areas (training modules). Thanks to the training, coordinators will gain or strengthen skills that will help them in their daily work. The project also provides for specialized training for a group of 100 people, who will later be able to train new coordinators. Training for leaders (trainers): Coordinators who have completed training for leaders (trainers) will co-lead/provide training for coordinators in their region as part of their traineeships. The project was divided into tasks: Activity 1 – Preparation of trainings and recruitment of participants, Activity 2 – Implementation of trainings, Activity 3 – Evaluation of project results on the basis of qualitative research (evaluation of trainings, satisfaction surveys of training participants and OPZ patients), preparation of a summary report and organisation of a seminar/workshop/exchange of experience and good practices for coordinators (networking). The beneficiary declares to carry out a State aid test in cases where it occurs.
