The main objective of the project is to support entities conducting medical activities and having concluded contracts such as primary health care with the National Health Fund in the implementation of health care coordination. The main objective will be achieved through specific objectives: equipping all participants implementing OK with appropriate competences and knowledge, taking part in the project, broadening the knowledge, competences and skills of staff in the field of organization and functioning of OK, disseminating good practices in the organization and substantive basis for the provision of services by POZ entities, which is necessary for the full implementation of tasks related to OK, increasing the number of POZ that provide services in the field of OK, thanks to appropriate education and training within the project, education of patients covered by OK in the field of diseases that affected them, expansion of the platform coordinated.nfz.gov.pl and HEALTH KNOWLEDGE TRANSFER HUB, thanks to which knowledge about the quality of services provided in the facility will increase, developing a tool based on the SCIROCCO Maturity Model for Integrated Care to assess the maturity of POZ institutions to provide OK, which will be aimed at developing a project of changes for POZ related to the implementation of OK, taking into account the justification, strategic plan and vision of a better OK. The project involves training staff ensuring effective implementation of tasks related to patient care in OK and employed in 1,500 entities conducting medical activities in POZ. The target group will be entities having signed a contract for the provision of services in the budget entrusted to OK in the POZ and entities that do not have such a contract concluded, but planning to sign a contract for the provision of services in the budget entrusted to OK in the POZ during the implementation of the project. The draft declares the application of the ‘do no significant harm’ principle to the environment (the DNSH principle). All products under the project will be available in accordance with Annex 2 to the Guidelines for the implementation of equality principles under the EU funds for 2021-2027. The project is in line with the Charter of Fundamental Rights of the European Union of 26 October 2012 (Journal of Laws No. Ooh, ooh, ooh. EU C 326, 26.10.2012, p. 391), as far as the manner of implementation and the scope of the project are concerned. The project consists of 15 tasks. Tasks in the project will lead to an increase in the number of entities with the concluded contract, and thus, equalization of the availability and quality of coordination of the provided benefits at one target level throughout the country, regardless of location. As part of the tasks, project participants will be equipped with appropriate competences and knowledge in the field of OK.
