Opportunity for a better tomorrow

The aim of the project is to increase the availability of social and health services for 97 people (68Ki29M) through the provision of comprehensive, home medical care and social support, and to increase the competences of staff (6Ki2M) providing social and health services in the period from 01.10.2024 to 31.03.2027. Taking into account the demographer's forecasts regarding the size of the population of 60+ in the Świętokrz region, measures should be taken to ensure the provision of social services to this group of recipients on a large scale. When planning social policy, it is necessary to develop social and health services dedicated to the elderly and the elderly, and to carry out prevention as part of active inclusion and health-promoting activation services. With the growing number of older people, it is becoming increasingly important to design social policies towards them and implement solutions that will ensure their dignified, safe and active ageing. The area of health care is one of the most important areas in the senior policy of the Świętokrz region. In 2050, the share of people aged 60 and over in the region will reach 44% (according to the Diagnosis of Accessibility, Kielce 2024). The applicant, running a medical facility CM Zdrowie and having extensive experience in providing long-term care services, diagnosed the need for comprehensive, multi-faceted medical care provided at home for K/M in need of support in everyday functioning, in particular for the elderly. The support offered in the project would be an opportunity for them to improve their health, increase their independence in everyday activities. The project would also make it possible to relieve caregivers of dependent K/M, organize family life, and enable them to take up or continue employment. The support also applies to caregivers of sick and dependent people. The implementation of the project will also contribute to reducing the number of unjustified hospitalizations of dependent persons/disabled persons and will increase the availability of health services used by dependent persons, as well as contribute to saving resources in the health system. The project has been planned and will be carried out on the basis of a local diagnosis of current social challenges and health needs of the inhabitants of the Ostrowiecki, Graz, Starachowice and Konecki districts. The problem is the creation of a comprehensive package of social and health services, which will be addressed to dependent people who need support in everyday functioning and their caregivers. The implementation of the project will contribute to the achievement of the goal of detail. ESO4.11(k) Enhancing equal and rapid access to quality, sustainable and affordable services(...) through the implementation of the tasks planned by the Applicant: 1. health services, including home long-term nursing care, examinations of specialist doctors, psychological support and medical telecare; 2. social services, including specialist services in the field of physical rehabilitation, respite care, care services, organization of health stays and advisory social care; 3. additional services for the rental of specialized equipment, minor repair services at home, workshops for actual carers; 4. equality and non-discrimination actions; 5. support for staff. The project demonstrates compliance with the principles of: −equality K and M according to the MINIMUM STANDARD: The applicant has planned the implementation of the project in such a way that there are no equality barriers at any stage of the project. In accordance with the principle, the project guarantees equal access to project support for all K/M. GD will be selected on the basis of clear recruitment criteria. Recruitment of personnel (K/M) to work in the project will be carried out in accordance with the principle of equality, taking into account only competence and professional experience.
