The aim of the project is to increase access to integrated, sustainable, high-quality community services responding to the diagnosed needs of the local community within the newly created CUS. A resident in one place will receive support without having to move to other units from the area of social and health prevention. Period of realization. project. 01.01.2025-31.07.2027 (31 months). Strategy of solution. Problems of Spod. Daleszyce Municipality 2021-2030 has shown that not all the needs of residents are yet sufficiently met, especially in the areas of elderly support, chronically ill people, disabilities, family support, health promotion/protection. As a result, the main recipients of CUS services will be children and adolescents, families with children with care and educational problems, ON and the elderly. There is a need to create services integrating the local community, stimulating social activity, which will shape the sense of community, willingness to cooperate, care for the affairs of the community, among others by launching neighborly services, the appointment of the Organizer of the local community. As a consequence of CUS' activities, the catalogue will be expanded and access to integrated services provided to the local community will be increased. CUS ensured coordination of social and health services provided in the Commune, among others through cooperation with organizational units of the Commune, NGOs. Implementation support in line with 4 milestones under 5 packages: 1) YOUTH - dedicated to children and schoolchildren up to 18 years of age. Existing services will be expanded to include: street educators (720h), compensatory activities for children, young people (900h), individual psychological support (800h), help with homework and leisure activities combined with activities aimed at shaping anti-discrimination attitudes based on, inter alia, gender, race, sexual orientation, gender identity, origin, etc. (1800h) carried out in the village halls in the commune. 2) FAMILY - assumes support for families with children mainly through workshops and classes increasing parental/educational competences (11 series of workshops-minimum 55h), self-help. support groups for parents (120h), extrajudicial mediation (90h), psychologist's help. (480 hours). The implementation of the package will contribute to: restore the family's ability to properly perform its functions, help parents overcome educational problems, improve the ability to solve problems in the family. Self-help will be conducive to environmental integration, bond building, cooperation and mutual assistance 3)SENIOR - addressed to people of the above-mentioned age. 65 years old The implementation of the package by launching the previously unavailable neighbourhood care service for 100 persons (43 200h - wed 1728h/m-c ) and giving them life envelopes (100pcs) will promote reconstruction, acquisition, maintenance of the ability to participate in local life and perform community roles by seniors; integrating the neighbourhood and strengthening neighbourly ties, stimulating civic participation, combating social exclusion. The fact that they are implemented by trusted persons will allow them to cover vulnerable customers, including those with intellectual disabilities, mental health disorders 4)HEALTH-provide residents with free access to rehabilitation services (7 500h - wed 300 h/m-c), speech therapists (280h) and 36 thousand km of transport service (approx. 1440 km/m-c) which are the most lacking in the commune. The implementation of the package will improve health and improve the quality of life of residents. 5)CUS EMPLOYEES - dedicated to 4 employees employed in the newly created CUS. They will be implemented: a study visit to a functioning CUS, training in effective and efficient communication with the client. The application for a minimum of 30% of the above-mentioned services will be commissioned by PES (PLN 1,727,700.00, which is 80% for RNM).
