Active despite cancer - continuation of the rehabilitation program of cancer patients aged 18-64 from Wielkopolska, implemented by OPEN S.A. in 2024-2028

Active despite cancer - the continuation of the rehabilitation programme for cancer patients aged 18-64 from Wielkopolska, implemented by OPEN S.A. in 2024-28, is an extension of the project implemented by the Applicant in 2021-23. The project will be implemented in partnership: -Alina Pienkowska Cancer Prevention and Epidemiology Centre in Poznań (OPEN), initiator of the identification of oncological rehabilitation as an important area of medical rehabilitation, -The People for People Social Foundation (LDL), which has been fighting for 35 years for a high level of treatment and oncological awareness. OPEN and LDL jointly implemented the project ‘Rehabilitation of cancer patients aged 18-64 from Wielkopolska, implemented by OPEN S.A. in 2021-2023’ (P21-23), of which OPEN was the initiator and originator. It was a pilot of comprehensive rehabilitation of patients/oncology patients in Wielkopolska. The implementation of the current project by OPEN and LDL will ensure, among others: -the unique strength of reaching the target group, based on trust resulting from the implementation of P21-23 -experienced substantive staff, composed of specialists creating standards of oncological rehabilitation - efficient organization and implementation of the project by an experienced team based on proven mechanisms of action - care for the quality of support, based on passion and sense of mission - OPEN motto "We are here to make oncological hospitals empty" - a high level of educational content, resulting from many years of LDL experience and belief that rehabilitation, as the third phase of prevention, is as important as treatment - LDL motto "Prophylaxis - treatment of human consciousness". Main objective of the project: Improvement of functional efficiency in not less than 30% of participants of the project (PO) during its implementation by providing rehabilitation support to 2,500 economically active and unemployed people from the Wielkopolskie voivodship aged 18-64 and support to 250 people from their surroundings a) Improving mental condition 15% UP, nutritional status 15% UP, activity and physical fitness 30% UP b) Raising or maintaining a high level of knowledge: -75% of people from the patient's environment who took part in education -85% of the medical staff of the program who took part in training c) Maintaining or returning to professional activity at least 850 UP. Project deadline: 1.09.2024 - 31.08.2028 Project activities: 1. Modification of the educational and information portal and carrying out a broad information campaign; 2. Start-up and retrofitting of the equipment of the Reference Centre and 3 branches and equipment of 1 branch; 3. Rehabilitation activities resulting from RPZ for 2500 people during or after cancer and 250 people from their immediate environment; 4. Training in physiotherapy, dietetics and psychology for 52 people from medical staff; 5. Drawing up a ‘Programme Performance Report’. The support will be implemented on the basis of the provisions of the Act of 27 August 2004 on health care services financed from public funds, in particular the Regulation of the Minister of Health of 6 November 2013 on guaranteed services in the field of therapeutic rehabilitation and in accordance with the Regional Health Programme “Rehabilitation programme for oncological patients aged 18-64 from the Wielkopolskie Voivodeship” (RPZ) [access criterion 3 - KD3]. The RPZ is an update of an earlier programme in this area, modified as a result of evaluation P21-23. The implementation of the project results from the diagnosis contained in the Health Needs Map [KD4] (more in section 3.9) Implementation of the project in the Reference Centre and 4 branches ensures reaching all residents of the province and determines the implementation of the project throughout Wielkopolska. The schedule of the project was rationally planned, using the experience from the implementation of P21-23. The whole assumptions of the project result from the P21-23 evaluation and are based on the experience of its implementation.
