The subject of the project is the implementation of activities in the area of medical rehabilitation facilitating the return to work or maintaining employment of 1243 people. (K-808,M-435) 18 years of age working or registered as unemployed in the Leszno subregion of the Wielkopolskie voivodship in the period from 01.05.2025 to 30.04.2028. The main objective of the project is to achieve improvement in each of the areas (in accordance with the RPZ) for which functional disorders negatively affecting the area of activity of project participants were detected in at least 950 persons participating in the project in the period from 01.05.2025 to 30.04.2028. The headline target will be achieved through specific objectives: Obtaining an improvement in functional efficiency in the area of musculoskeletal and spinal disorders in at least 1,000 people. Reduction in pain intensity in at least 1.070 pers. An increase in the frequency of physical activity in at least 300 people. Increasing the level of knowledge about proper health behaviors, prevention of musculoskeletal disorders and consolidation of normal motor habits and ergonomics in the workplace in 1,150 people. The main problem faced by potential project participants is musculoskeletal disorders and spinal diseases that result in reduced full fitness in moving and performing everyday activities, in particular limiting the performance of professional work and, consequently, absenteeism as a result of own disease at the workplace. According to the analysis of the needs maps for 2022-2026, the share of patients diagnosed with osteoarticular and muscular disease was the highest in Wielkopolska - 67%. The target group is Pw. 18 years of age working or registered as unemployed in the Leszno subregion with diseases or injuries of the osteoarticular, muscular and connective tissue along with pain in the spine. Therapeutic activities will be covered by 1243 pers. (K-808,M-435). The residents of the Leszno subregion who are eligible to take part in the project will be covered by information and education activities. Over the 10 years (2013-2023), according to ZUS, the number of first-time judgments entitling to rehab services has successively increased from 85.1 thousand in 2013 to 115.7 thousand in 2023. Rehab benefits in connection with osteoarticular and muscular diseases were always the highest percentage. The project will be implemented in a partnership formula. The applicant will carry out information and education activities (degree 1), therapeutic activities (degree 2) and recreational and sports activities (degree 4). Classes in health education (degree 3) will be carried out by the Partner. The tasks assumed in the project will translate into achieving the assumed results of the project, i.e. the number of people who, thanks to support in the area of health, took up work or continued employment (K-808,M-435) and the number of people who improved their health (K-715,M-385). The material effects that will be obtained as a result of the implementation of the tasks assumed in the project will be the number of people covered by support in the field of health (K-808,M-435) and the number of implemented health programs relevant to the health needs of the region, including employers (1). The support planned under the project will be implemented on the basis of the provisions of paragraph of 27 August 2004 on health care services financed from public funds. The project is in accordance with the Convention on the Rights of Persons with Disabilities of 13.12.2012. at no stage of the project are there any contradictions between the provisions of the project and the requirements of the Convention. Furthermore, the requirements of the Convention are not neutral with regard to the scope and content of the project in question. Persons registered as unemployed (not receiving support from the competent labour market institution) covered by support under the RPZ will be informed by the Applicant and the Partner at least about the possibility of obtaining support in projects in the field of professional activation implemented under the programmes.
