"Wielkopolskie telecentre care"

The main objective of the project is to improve access to better quality social services, including telecare, through the development of comprehensive services in the local environment for at least 5,000 older people (60+) and at least 1,000 informal carers implemented in the period from 01.07.2023 to 30.06.2029. The project will contribute to improving access and coordination of services for the elderly and carers in at least 70 municipalities in Wielkopolska and to creating a Regional Senior Care System at the voivodship level. The target group supported will be: 1) elderly people aged 60+, 2) informal carers, 3) staff of institutions of the regional social policy system (including OPS, CUS, Municipal Offices). The project assumes the launch and strengthening of coordination at the level of the voivodship of the Regional Senior Care System, under which comprehensive services for the elderly and carers will be organized and provided, including a telecare service (organised and provided by the Regional Centre for Social Policy in Poznań, hereinafter ROPS), which will be supplemented by services provided in the local environment (organised by municipalities as part of the grant and own funds granted). Support will also be provided to the staff of institutions from the municipalities selected as part of the call (e.g. training, study visits, conferences). 1) Regional Telecare System consisting of: a) Telecentre with paramedics (24/7), which will provide telecare services (e.g. using bands) for the elderly as a form of prevention aimed at maintaining the independence of seniors and as a form of supplementing environmental services b) Social Centre providing social support, consisting of: i. Social Telecentre integrated with the telecentre with paramedics, providing counselling services, including specialist (e.g. psychological) for the elderly and carers using the telecare system ii. The above actions will be supported by telecare assistants. iii. training and advisory support for carers 2) municipal support systems for seniors – assuming the development of services provided on a local Wednesday for the elderly and carers, including, among others, the launch of Senior Support Centres and the function of a coordinator of services in the area of support for the elderly organised by municipalities under the grant granted (maximum EUR 200 000). Effects of interventions: At least 5,000 older people will receive comprehensive services, including telecare services with paramedics and social services (organised by ROPS) and accompanying services organised by the municipality (e.g. coordinator support, assistant, care and neighbourhood services). At least 1,000 carers will receive support, including training and advisory services (organised by ROPS) and services organised at municipal level (e.g. coordinator support, respite care). Trained carers will provide services in municipalities. Creation of at least 70 municipal support systems for seniors (excluding municipalities supported under the ITI project) providing for the development of services for the elderly and carers by launching Senior Support Centres with 140 places for the provision of services in the local community and the function of service coordinators in the area of support for the elderly (e.g. human resources support, provision of tools and working methods). The project contributes to the achievement of the objective through the implementation of key indicators, it is planned to achieve: • OK. 46.17% of the indicator target: number of people covered by services provided in the local community in the programme • approx. 64.89% of the target for the indicator: number of people providing services in the local community thanks to the support in the program • approx. 9.09% of the target value for the indicator: number of places of service created in the community
