The project will be implemented in the period from 01.07.2023 to 30.06.2026. The project will be implemented in the Wielkopolskie Voivodship, including the seat of the Regional Centre for Social Policy in Poznań (hereinafter: ROPS), ul. Nowowiejski 11, (61-731) Poznań as the Project Leader and the seat of 7 Wielkopolska communes - Partners who expressed their readiness to create the seat of the Social Services Centre (hereinafter referred to as CUS) in the commune: Dopiewo, Szydłowo, Śrem, Swarzędz, Rokietnica, Pleszew and Pniewy. The aim of the project is to increase access to a wide and comprehensive offer of social services provided in the local community under the CUS for 1400 Wielkopolska residents living in 7 communes – project partners (200 people in each of 7 communes), in the period from 01.07.2023 to 30.06.2026. In addition, the aim of the ROPS activities in Poznań is to strengthen 7 OPS and 70 employees of regional social policy institutions in the process of preparation for transformation, transformation of the social welfare centre (OPS) into a centre for social services (CUS) and development of social services and their quality. The target groups of the project will be: 1) residents of 7 communes, including in particular: - people in need of support in everyday functioning, including with mental problems and their surroundings; - people at risk of poverty and social exclusion, older, with limited efficiency, including NSAs, with a high social risk category (unemployed, poor, leaving institutions); - people at risk of housing exclusion; - persons eligible for prophylaxis, treatment or health rehabilitation due to age or health condition; - working poor, - people exposed to social exclusion, among others due to addictions or exposure to violence. 2) staff of institutions of the regional social policy and health system, including in particular employees of OPS/CUS. The project has two main components: 1) Actions implemented by the Leader (ROPS) supporting municipalities and OPS in the process of preparation for transformation, transformation of the social welfare centre (OPS) into a social services centre (CUS) and development of social services and their quality, addressed primarily to 7 partner municipalities, but also to other Wielkopolska municipalities in which CUS already operates or which express readiness to appoint it. ROPS support will include, among others, educational and information activities, specialist advice, activities supporting OPS/CUS staff in the implementation of tasks, study visits, trainings, workshops. The activities will be carried out by the Leader's staff and external specialists. 2) Actions implemented in 7 communes - Partners aimed at transforming OPS into CUS and launching within it social services provided in the local community within CUS for 1400 Wielkopolska (200 people in each of 7 communes). The activities of the Partners will include in particular: - carrying out a diagnosis of the needs of commune residents in terms of scarce social services; - development based on the diagnosis of the social services program and selection of 4 services to be launched as part of the project for each of the CUS; - conversion of OPS into CUS; - launch in each of the 7 CUS, 4 social services provided in the local community responding to the needs of residents. It is assumed that the services will be provided in accordance with the assumptions of deinstitutionalisation (DI) and the conditions formulated in the area-related guidelines for ESF+. The most important effects of the intervention: -At least 1400 people will receive high-quality, accessible services provided in the local community, -A minimum of 70 employees of the staff of the institutions of the regional social policy system will be supported in the development of CUS and the provision of high-quality social services, - support will be provided to 7 public administration entities or public services at national, regional or local level - 70 places for the provision of services in the local community will be created, - 100 people will provide services in the local community thanks to the support in the program, -
