Neighbourly assistance - development of a system of social services provided in the local community for the residents of Poznań

The main objective of the project is to develop a comprehensive system of social services for the inhabitants of Poznań by creating in the area of 1.12.2023-31.12.2026r.202 places for the provision of services in the local community. 1166UP (Access Criterion 1/ KD1) will use the services. The target group (GD) of the project is 1166 natural persons who are studying, working or residing in Poznań (within the meaning of the CC), including: A. 1066 persons in need of support in everyday functioning, who due to age, health condition or disability require care / support due to the inability to independently perform at least 1 of the basic activities of everyday life, including the elderly / OST and people with disabilities / OzN (min. 455 OzN). B. 100 persons are the actual guardians of the above-mentioned persons/OF (75K, 25M). The goal will be achieved through the creation of 202 community service places. As a result of the implementation of individual tasks in the project will be created: 1. 40 places of care for personal assistant OzN/AOON - Zad.1 2. 40 Neighborhood Care Facilities - Zad.2 3. 21 places of care in the place of residence - Zad.3 4. 7 places of specialized care services in the place of residence - Zad.4 5. 51 respite care places - Zad.5 and 10 6. 18 places in assisted apartments - Zad.7 (Premium Criterion 1 and 2/ KP1 and KP2) 7. 22 places in the Neighborhood Senior Club - Zad.8. 8. and educational support for 100 OF (3 places) - Zad.6. The total number of people covered by the above-mentioned services provided in the local community in the program will amount to 1,046 people. The project will be implemented in partnership with the City of Poznań/UMP (Faculty of Health and Social Affairs/ WZiSS – Zad. 3-6 and 8; Office for Project Coordination and Revitalization of the City/BKPiRM) and the Sowelo Foundation (Zad. 1) - subject of social economy / KP3). Some of the tasks will be carried out by UMP organisational units: MOPR (ref. 2, 7 and 9) and DPS at ul. Konarski 11/13 (Zad. 10). The scope of social services planned in the project concerns only services provided in the local community within the meaning of the Guidelines for the implementation of projects involving the European Social Fund Plus in regional programmes for 2021-2027 (KD2). The activities undertaken by the Applicant and the Project Partner will be consistent with the Wielkopolska Plan for the Deinstitutionalisation and Development of Social Services (RPDI). The project is addressed to the elderly, OzN and OF. The support offered under the project will be individualized and tailored to the needs of the person requiring support. The implementation of the project will contribute to the improvement and development of services provided in the local community in the City of Poznań. The design was designed according to the idea of universal design. The proposal provides for comprehensive support tailored to the needs of the target group. After the conclusion of the project co-financing agreement, the applicant will inform local social assistance institutions about the activities planned in the project. For this purpose, the model information provided in the supporting materials will be used. The applicant shall represent the municipality, the OPS and the social affairs department operating in the intervention area.
