The project is addressed to 50 (28K and 22M) people in need of support in everyday functioning, i.e. people who, due to age, health condition or disability, require care or support due to the inability to independently perform at least one of the basic activities of everyday life, local community living (within the meaning of the Civil Code) in the Stawiszyn Commune in the Wielkopolskie Voivodeship. Within the framework of the project, support is aimed in particular at: severely or moderately disabled; - with a coupled disability; - with mental illnesses; with an intellectual disability; with overall developmental disorders (as defined in the International Statistical Classification of Diseases and Health Problems ICD10); - benefiting from the FE PŻ programme; living alone; The GD provides for: 10 OzN (6K4M) and 50 (28K and 22M) pers. from rural areas (DEGURBA3). The aim of the project is to: Improving access to better quality social services for 50 (28K and 22M) people in need of support in everyday functioning, i.e. people who, due to age, health condition or disability, require care or support due to the inability to independently perform at least one of the basic activities of everyday life, the local community residing (in the meaning of the CC) in the Stawiszyn Commune in WW, through stationary care services within the established seniors' club in the period 05.2024-05.2026. Actions: 1. Creation of Senior Club 2. Organization of classes within the Senior Club: SOCIAL, HEALTHY, ARTISTIC, LEGAL AND PSYCHOLOGICAL WORKSHOPS / TOUR AND 3. Organization of classes within the Senior Club: SOCIAL, HEALTHY, ARTISTIC, LEGAL AND PSYCHOLOGICAL WORKSHOP / TOUR II 4. Cultural trips / TURA I 5. Cultural trips / TURA II As part of the project, care services in the form of residential day care will be provided in the municipality of Stawiszyn within the framework of the established senior club "Senior with vigor". As part of the established senior club, dependent people will have the opportunity to meet, establish contacts, participate in various active forms of support that will affect their development and well-being. The measures taken will counteract the loneliness and marginalisation of seniors in the Stawiszyn Commune. Result: increase in the number of people benefiting from better quality health and social services - 50 people The project will be implemented in partnership with the Municipality and the City of Stawiszyn. Value of the project: 775 147,80 zł Dof: PLN 727 147.80 WW: The project meets the access criteria by: 1. Implementation period until 31.05.2026. 2. Services are provided in the local community - LOCAL ANALYSIS OF NEEDS described in point. 3.4.2. 3. WN submits 1 application under the call, 4th project leads to an increase in the number of places for the provision of services in the local community - the applicant declares that 25 places for the provision of care services in the local community will be launched, which will allow a total of 50 people to be covered by services provided in the local community. Last year and now the Partner provides 30 places for 30 people, but there is a demand elsewhere in the Municipality for additional places, which is why the project is a response to this demand. Thus, the number of places will be increased by 25 and a total of 55, and the number of people supported will be increased by 50 and a total of 80. 5.
