"Support for the deinstitutionalization of foster care in the Poznań subregion"

The aim of the project is to: "Support for the deinstitutionalisation of foster care in the Poznań subregion" is to increase the availability and broaden the scope of support services for 1162 people, i.e. children and young people in foster care (PZ) and to deepen/extend cooperation under 6 district foster care systems and their 80 employees performing tasks in the field of family support and foster care system in the period from 1.10.2023 to 30.06.2029. The project will be implemented in the city of Poznań and in the following districts: Oborniki, Poznań, Szamotuły, Średzki and Śrem. The target group is: a) children and young people in foster care, including institutional and family type, their biological families and their surroundings b) independent and independent pupils of foster care c) entities and persons performing tasks in the field of family support and foster care system in accordance with the Act on family support and foster care system, including: - persons performing the function and candidates to perform the function of a foster family, including a related, unrelated, professional, specialist, family children's home and a family-type care and educational institution and their surroundings - persons performing the function of independent caregiver - staff of the institution of the regional family support system and foster care system, including employees of units performing tasks in the area of foster care - other entities of institutional care for children and young people, e.g.: socialization, intervention or specialist-therapeutic care and upbringing institutions, within the meaning of the Act of 9 June 2011 on family support and foster care system or in another multi-person institution, round-the-clock stay or care - other public, private and social stakeholders who are key for the process of independent education of pupils - e.g. employers, representatives of local government units - local communities. The following activities will be organised as part of the project: a) services for people in foster care (children and young people), including but not limited to: - in-depth diagnoses, - supporting development, compensating for educational and social delays, including speech therapy, physiotherapy, psychotherapy, trauma therapy, etc. - preparing for independence, among others skills workshops, self-reliance trainings, support groups, etc. - supporting in the process of emancipation, including: housing with support, support in obtaining employment, support groups - psycho-prophylactic and violence prevention b) services for people performing the functions of a foster family: related, non-professional and professional (including ambulance and professional family), as well as family homes of the child; in addition, for the caregivers of an independent person, biological families of children staying in the PZ and their surroundings (including grandparents, other relatives who can provide support for people who care for children), c) support for the staff of the institutions of the regional family support system and the PZ system, including primarily employees of units performing tasks in the area of the PZ, including, among others: - support in the implementation of the model of early emancipation of PZ pupils and other activities improving the quality of services addressed to target groups - networking, exchange of experience, knowledge, communication; - training support. The most important effects of the above intervention include: - 1162 children and young people in foster care will receive family support services and a foster care system, which will allow them to be better prepared for independent living, - 26 foster families will be covered by support enabling them to better perform their role, - 6 districts will be covered by support in the field of deinstitutionalization (DI) of foster care, - 80 employees of the family support and foster care system will be covered by support in strengthening competences to perform tasks in accordance with the
