Objective of the project: Combating social exclusion and social activation of 100 people (60Ki40M) at risk of poverty and social exclusion through the implementation of social inclusion measures, i.e. 1. Social Inclusion Training, 2. Psychological support, 3. Streetworker support, 4. Support Assistant for Persons with Disabilities, 5. Manual courses - e.g. tailoring, pottery, floristry, macrames, 6. Coaching and 7. Preventive anti-addiction workshops, 8. Physical Activity Workshops, 9. Cognitive trips, 10. Organization of social events - building interpersonal relations in society, 11. Educational broker. leading to improved functioning in society in areas, i.e. municipalities: Stawiszyn, Mycielin (Gmina Zagr with permanent marginalisation), Ostrów Wlkp. in the period from 01.01.2025 to 30.06.2026. Target group: Individuals, families, local communities at risk of or affected by marginalisation and social exclusion – a total of 100 people (70 adults and 30 children/youth), including: -persons with disabilities (including coupled persons with disabilities; with mental illnesses and problems; with an intellectual disability; with comprehensive developmental disorders) (12) - people affected by long-term or severe disease (18) - people from families with dysfunctional (e.g. affected by poverty, domestic violence, alcohol) (40) - elderly people 55+ (30) who learn or live according to the Central Committee on the OSI indicated in Annex 3 to the CPR: communes Stawiszyn, Mycielin, Ostrów Wlkp Project tasks: Indicated for the main purpose from 1-11. Effects: The project is part of the ESF+.CP4.L specific objective: Support the social inclusion of people at risk of poverty or social exclusion, including the most deprived and children, by providing measures to support the social inclusion of the target group. Target group i.e. 100 people will be covered by tailor-made project activities that will ensure the prevention of social exclusion and activate the socially supported people. The project will have a positive impact on reducing the phenomenon of social exclusion in the area of project implementation. Developed attitudes and skills will allow you to cope with difficult life situations and improve your own life situation. Result indicator: The number of people whose social situation has improved after leaving the program will be reached 100% for 100 people. Criteria: The project meets the following criteria: Access: 1. The CA submits 1 application under call 2. Project deadline: 01.01.2025 - 30.06.2026 r. 3. GD are individuals, families, local communities at risk of or affected by marginalisation and social exclusion. 4. WN has planned actions: activation, integration, education, information, dissemination, prevention, culture, sports, recreation. 5. The actions implemented shall be consistent with the WPDI. Preferential: 1. The project is implemented in the municipality of Mycielin, 2. Project implemented in partnership - public sector (Municipality) with PES/NGO (Association), 3. The project will involve 2 streetworkers with the required experience with GD. 4. At least 1 psychologist with at least 2 years of experience with GD will be involved in the project.
