The subject of the project will be the implementation of transformation processes in the Czech Republic, in particular the implementation of transformation plans, participation in the implementation of measures from the National DI Action Plan, as well as support in the broader sense of the implementation of deinstitutionalization and transformation of social services in the Czech Republic by regions, providers, users, the public and other partners through piloting the DI management and coordination system and monitoring.
