The aim of the project is to improve the availability of measures at local level to prevent aggravation of the need for assistance or support. As a result of the project, 400 people with need for assistance and support and/or their relatives have received counselling services and their coping in the home environment has improved thanks to better access to social and other support services that meet their needs. A cooperation model has been developed with local governments and other necessary parties to support those in need, which is expanding across Estonia.
