The project applicant - Rezekne State City Municipality (hereinafter - PI, also the municipality). The institution responsible for the implementation of the project is the Rezekne State City Municipality Administration "Social Service" (hereinafter - the Administration). The aim of the project is to increase the availability of community-based social services (hereinafter - SBSP) and the provision of community-based social services to the target group persons referred to in Cabinet Regulation No 593: adult persons with mental disorders for whom a disability group I or II has been determined (hereinafter - persons with GRT); children with functional impairments who have a certain disability and who live in families (hereinafter: children with FT) in the territory of the municipality. The municipality is developing the SBSP, making increasing its share a priority for the municipality. Main activities of the project:1. assessment of individual needs and development of a plan for the provision of social rehabilitation, social care, child development or personal or family support services (hereinafter - the support plan) to persons of the target group;2. provision of community-based social services to persons of the target group;3. measures for ensuring communication and visual identity requirements;4. project management and ensuring its implementation. Planned results of the project: The planned project activities are the provision of SBSP to 15 target group persons. On the basis of the municipality's existing SBSP infrastructure, the number of SBSP beneficiaries will be increased:1. by 10 places - for persons with mental disabilities (hereinafter - GRT) in the Multifunctional Social Services Centre of the Administration (service of the Day Care Centre and Specialized Workshop);2. by 5 places - for children with functional disorders (hereinafter - FT) in the Day Care and Rehabilitation Centre "Spārīte" of the Social Services Division of the Administration (service of the Day Care Centre).The total planned cost of the project is EUR 475 050.00, including the planned financing of the European Social Fund Plus (hereinafter - ESF+) in the amount of EUR 403 792.50 and the planned co-financing of the local government budget in the amount of EUR 71 257.50. The project will be implemented between June 2025 and September 2027.
