The aging of the population is a challenge. Independent living at home of the elderly is supported by encouraging the elderly to independently strengthen their health and functional capacity. Municipalities should increase counseling related to this, increase the search for older people belonging to identified risk groups, and direct them to activities that promote well-being and functional capacity. Functional capacity and living at home are promoted by addressing risk factors that decrease of functional capacity at the rich time. Mobility problems are the most common risk factor and are related to functional impairment, health and physical condition, and the fear of falling. It is important to recognize mobility problems early enough and they should be addressed. Mobility problems are often addressed only when the person has already fallen, has a history of falling, or reports difficulties in mobility. In this way, the possibility of preventing a fall is bypassed. Sufficiently sensitive measures should be used early enough to identify predictive factors. Physical activity is an important factor in preventing mobility problems and supporting coping at home. Technological and game solutions are one way to increase physical activity. Exergaming have been found to motivate and increase commitment to rehabilitation, improve the mobility of elderly people, and improve the performance of daily activities and independent coping. Exergameing have been mentioned in the KATI pre-examination project as one application that supports living at home. Likewise, measuring devices for monitoring activity and functional capacity, as well as wearable technology. Despite the potential, technology is used quite a bit in supporting living at home and promoting functional capacity. The transition to welfare areas brings big changes to the service structure. The responsibility for preventive services lies with the welfare regions, but the implementation lies with the municipalities. In order to respond to this change, municipalities need new ways to implement preventive activities. Technological solutions in collecting predictive information and supporting elderly people living at home - the purpose of the project is to promote proactive actions by utilizing digital and technological solutions to support elderly people living at home over long distances. This is aimed at by jointly developing and piloting with various actors a proactive model that utilizes digital and technological solutions. At the beginning of the project, the representatives of various actors are mapped into the development work group (eg pilot municipalities, councils for the elderly, organizations, parich, sports and cultural activities, welfare area, cooperation projects). The model uses digital and technological solutions as needed. Co-development is implemented through workshop work. The developed proactive model is being piloted in cooperating municipalities and its effectiveness is evaluated. The result of the project is a proactive activity model that supports the functional capacity of the elderly and survival at home using digital and technological solutions. In addition, awareness of digital and technological solutions among different operators is increasing. As a long-term result, the elderly in Lapland will live at home for longer with the help of proactive measures and digital and technological solutions. The operating model developed in the project is intended to be implemented in the entire welfare area.
