The project proposal aims to continue the process of preparation, the removal of a minimum of 2910 adults with disabilities, accommodated in 41 specialized institutions (SI) and about 290 adults with disabilities residing in 11 State Psychiatric Hospitals (PSHs) for social indications and their placement in a suitable place to live in the community. The following activities will be implemented: 1. Creation and training of teams to carry out preparation for removal, psychological counseling, assessment of specific medical needs, assessment of the need for intensive support from specialists; 2. Preparing assessments of the needs of persons with disabilities and plans for their removal with recommendations for support in case of placement in the new residential care services; 3. Assessment of the needs of persons permanently housed in state psychiatric hospitals and their referral to appropriate social services. Preparation of persons for removal from state psychiatric hospitals; 4. Ensuring supervision of the teams involved in the evaluations and the development of the plans for the withdrawal of the users, as well as those providing social services in the specialized institutions, until their final closure; 5. Preparation of a specialized program for preparation for independent living, according to the main specifics of the relevant group of disabled people. Planning the activities for their actual output. Preparation of the exit teams for the implementation of the program, supervision of its implementation; 6. Preparation of a plan for closing the specialized institution.
