The project proposal "Care at home in the municipality of Ardino" is aimed at providing support in a home environment to the target group - adults unable to care for themselves and people with disabilities. Its implementation will complement and upgrade other social projects implemented by the municipality of Ardino, such as "Patronage care+" and will give a wider spectrum and scope of activities in the direction of providing social and integrated health and social services in a home environment, even in a setting of pandemic situation. A minimum of 135 representatives of the target group - adults unable to care for themselves and people with disabilities - will be covered. The services will be provided to those in need for a period of 22 months, by implementing the following activities: Provision of hourly mobile integrated health and social services at homes and psychological support. Purchase and delivery of food, foodstuffs and basic necessities, payment of household bills and provision of administrative and household services with consumer funds.Provision of information on prevention and prevention of health and socially significant diseases and assistance, consultations with specialists if necessary. The provision of the service will be assisted by providing transport to the staff providing the services to/from the individuals' homes. The implementation of these activities will be realized by forming a team of specialists to provide the service - domestic helpers, nurses, social worker, psychologist depending on the individual needs of the users of the project, identified on the basis of a needs assessment. The appointed persons will be supported by providing the necessary training (introductory/upgrading) for the performance of their activities, as well as by conducting supervision.
