"CARE AT HOME IN ASSENOVGRAD MUNICIPALITY"

The main goal of the project proposal is to upgrade the model for hourly mobile integrated health and social services - patronage care for needy persons with disabilities and elderly people in the municipality of Asenovgrad to improve the quality of life and their opportunities for social inclusion. A team of specialists will be hired - nurses, domestic helpers, social workers, social activities assistant - dispatcher, psychologists, nutritionist doctor, endocrinologist doctor, cardiologist doctor, neurologist doctor) who will provide various health and social support activities. Support is provided for the personnel directly involved in the provision of health and social services - training and supervision. During the implementation of the activities, access to long-term mobile and integrated health and social care will be covered and provided, according to the specific needs of 309 persons from the target groups, namely: Elderly people unable to care for themselves and people with disabilities. They will receive psychological and social support and counseling, as well as information on prevention and prophylaxis of socially significant diseases from specialist doctors. Home care will be provided for 22 months within the project. The project proposal was developed in accordance with Priority 2 "Social inclusion and equal opportunities", specific objective 3. The project will build on the activities implemented and the results achieved under other projects implemented in the municipality of Asenovgrad, which are financed with funds from the state budget and under the Operational Program "Development of Human Resources" 2014-2021. The project contributes to the implementation of Specific Recommendation 4 for 2019 "...To take measures for social inclusion by improving access to integrated social services...". The project contributes to the implementation of principle 18 "Long-term care" of the European Pillar of Social Rights.
