The activities of the project will provide support in a home environment for persons with disabilities and elderly people dependent on care. The possibilities for the provision of social and integrated health and social services in the home environment will be expanded, as well as the provision of psychological support. The project will support the improvement of equal and timely access to quality and sustainable services at an affordable price, By starting the activities of providing the services in a home environment from 03.02.2023, the continuity of the service will be ensured for the elderly and people with disabilities, since the activities under the Project " Patronage care + "ends its activities on 04.02.2023 incl.The project will provide hourly mobile health and social services and psychological support in a home environment for a period of 23 months to 59 persons from the target groups. 16.5 positions will be opened for the position of paramedics, a nurse for 4 hours a day, a psychologist for 4 hours a day and a rehabilitator for 8 hours a day. The length of employment of the hired staff will be 23 months. Inexperienced hires will be provided with project-specific induction and refresher training, and group supervision will be provided for all hires.Transportation will be provided to all persons hired under the project, as well as a car that will be available to a nurse, psychologist and rehabilitator on a daily basis. Information campaigns will be organized regarding health prevention and prevention, including consultations with nutritionists and/or others specialists, depending on the individual needs of the persons.
