The project proposal envisages the provision of integrated social and health services, provision of support in a home environment for persons with disabilities and the elderly on the territory of Borovan municipality. The possibilities for providing social and integrated health and social services in a home environment to a minimum of 36 users will be expanded. The target groups that will be included in the project are: elderly people unable to care for themselves and persons with disabilities. Project activities are:1. Provision of hourly mobile integrated health and social services at homes and psychological support for elderly people unable to care for themselves and persons with disabilities. 2. Delivery of food, foodstuffs and basic necessities, as well as providing transport to the personnel providing the services from/to the homes of the individuals. Paying household bills, requesting and receiving urgent administrative and household services (with user funds) is also allowed. 3. Providing the persons from the target group with information on prevention and prevention of health and socially significant diseases, including consultations with a nutritionist, etc. specialists if necessary. 4. Providing supervision and training to employees providing hourly mobile integrated health and social services in a home environment. The current project proposal will build on the implemented activities and achieved results of the successfully implemented projects as follows: "Patronage care - Component 2" and 3 ", "Patronage care +" and "PG + Component 2" The implementation of the project activities will be realized by forming a team of specialists, domestic helpers, a medical person, a psychologist, etc., who, on the basis of an assessment of the needs of each user, will take care of the target groups in home conditions. Social assessments will be prepared of all users. The provision of the service will be assisted by the provision of materials and staff transport.
