The project proposal envisages the provision of hourly mobile integrated health and social services at homes and psychological support for the elderly, dependent on care and persons with disabilities; Delivery of food, foodstuffs and basic necessities, as well as providing transport for the staff providing the services from/to the homes of the individuals; Paying household bills, requesting and receiving urgent administrative and household services (with user funds); Providing the target group with information on prevention and prevention of health and socially significant diseases, including nutrition consultant, etc. specialists if necessary; Provide supervision and training to staff providing hourly mobile integrated health and social care services in a home environment. The services fulfill a preventive role and promote social cohesion, they are expressed in the provision of personalized assistance with the aim of facilitating social inclusion and the protection of fundamental rights. These include assistance to overcome personal difficulties or crises, activities aimed at ensuring that the persons affected have the necessary abilities for their full inclusion in society (rehabilitation), and in particular in the labor market (training, reintegration on the labor market). These services complement and support the family's role in the care provided, including activities to integrate people with long-term health problems or disabilities. The project provides an opportunity to ensure the employment of employed persons and their realization on the labor market, as well as to increase their qualifications.
