The project will provide home environment support for people with disabilities and elderly people dependent on external care. Activities will cover a minimum of 398 people, representatives of the target group of adults unable to care for themselves and people with disabilities.The project will build upon the implemented activities and the results achieved under the previous successfully implemented projects of Gabrovo Municipality under the procedures "Patronage care for the elderly and perpeople with disabilities" and "Patronage care +”. The support will be provided for the representatives of the target groups through the implementation of the following activities: 1.Provision of hourly mobile integrated health and social services at homes and psychological support for elderly people unable to care for themselves and people with disabilities; 2.Delivery of food, food products and basic necessities, provision of transport for the staff providing the services to/from the individuals' homes, payment of household bills, requesting and receiving urgent administrative and household services (with user funds); 3.Providing information about prevention and prophylaxis of health and socially significant diseases, incl. consultations with a nutritionist, and other specialists; 4.Providing supervision and training to employees, who realize hourly mobile integrated health and social home services. The implementation of the activities will be realized by a team of specialists: nurses, psychologists, social workers, managers of subsidiary activities, coordinator of the service, rehabilitators, physiotherapists, orderlies, drivers and specialists, depending on the individual needs of the people identified by the basic needs assessment. The appointed people will be supported by providing the necessary training (introductory/upgrading) for the performance of their activities, in cases where they have not undergone similar or the same, as well as by supervision .
