The implementation of the project proposal "Care at home - General Toshevo municipality" aims to create a sustainable model for the provision of integrated health and social services in a home environment for the elderly, dependent on care and persons with disabilities and at risk of social isolation. Hourly mobile integrated health and social services and psychological support will be provided; Delivery of food, food products and basic necessities, providing, if necessary, transportation of the personnel providing the services from/to the homes of the individuals, payment of household bills (with the funds of the users), requesting and receiving urgent administrative services; Information will be provided on prevention and prevention of health and socially significant diseases. It is planned to appoint 33 people to carry out the planned activities: a social activities associate, a social worker, home health workers 26 people, a driver, a psychologist, nurses/paramedics - 2 people and a technical partner. In implementation of the project activities, introductory and upgrading training and supervision of employees providing hourly mobile integrated health and social services in a home environment will be provided.
