This project will build on the implemented activities and the results achieved under the successfully implemented projects of the municipality under the procedures "Patronage care for elderly people and persons with disabilities" and "Patronage care +". Support will be provided for the representatives of the target groups through the implementation of the following activities: 1. Provision of hourly mobile integrated health and social services at home and psychological support. 2. Delivery of food, food and essential products, payment of utility bills, requesting and receiving urgent administrative and household services with funds of consumers. 3. Providing the target group with information on prevention and prevention of health and socially significant diseases, including consultations with a nutritionist, etc. Specialists if necessary.The project proposal is aimed at providing support at home for people with disabilities and elderly people dependent on care on the territory of the municipality. The opportunities for providing social and integrated health and social services at home will be expanded, incl. And in the context of pandemic situations such as the activities envisaged under the project will cover at least 211 persons, representatives of the target group - adults unable to self-service and people with disabilities. The activities will be implemented by forming a team of specialists such as domestic helpers, a medical practitioner, a psychologist and others depending on the individual needs of the persons identified on the basis of a needs assessment. The appointed persons will be supported by providing the necessary training for the implementation of their activities (introductory / upgrading), as well as by conducting supervision. The provision of the service will be supported by providing transport to the staff providing the services from/to the homes of the persons.
