The implementation of the project proposal "Care at home in the Municipality of Dobrich" aims to create a sustainable model for the provision of integrated health and social services in a home environment for elderly people unable to care for themselves and persons with disabilities.Activities are the provision of hourly integrated health and social services, psychological support, subscription cards for public transport, training and supervision of the staff.A team will be appointed to provide the services in a home environment: a social worker and a nurse, who will organize, coordinate and supervises the implementation of the activities,will prepare the individual assessments of the needs of health care, social and/or psychological support, weekly schedules for the visits and will maintain the necessary documentation.To provide the services, the following will be appointed 7 nurses, 1 psychologist and 1 rehabilitator, social worker 1 full-time person; personal assistants for up to 92 persons, full or part-time.The services will be provided on the basis of prepared individual assessments of the needs for health care,social and/or psychological support.To deal with psychological problems arising as a result of social isolation,as well as for support to lead an independent lifestyle, psychological support will be provided to each user with a stated need and a need assessment prepared.Nurses will provide health care and medical monitoring depending on the needs of the person and the treatment prescribed by the personal physician. Information on prevention and prevention of health and socially significant diseases will be provided by honey.nurses,a psychologist and a rehabilitator.Personal assistants will assist individuals with activities of a domestic and social nature, with the delivery of food products and products of first necessity,payment of household bills,requesting and receiving urgent administrative and household services (with user funds) .
