The main priority of the project proposal "Care at home in the municipality of Dimitrovgrad" is the development and increase of opportunities to improve the quality of life by providing individualized support for persons with disabilities and elderly people dependent on care, through the provision of integrated health and social services in home environment on the territory of all settlements in the municipality, incl. and in the context of pandemic situations. For this purpose, the functioning of the Unit for care at home is foreseen, in which a team of health specialists and specialists in social activities will work to provide hourly mobile health and social services in a home environment. Recruited staff will undergo training aimed at building appropriate capacity to provide home care services. The Unit's team will carry out an individual assessment to identify the specific needs and requirements of each user, on the basis of which the target groups will be provided with individual specialized support, including social and household activities, medical care, rehabilitation, social and psychological support, delivery of food and medicines of first necessity, payment of household bills, assistance in applying for urgent administrative and household services, promotion, prevention and counseling of consumers for socially significant diseases. The leading principle in the implementation of the project activities will be their binding to the specific needs and requirements of the persons from the target groups to overcome social isolation and ensure better living conditions.
