The project "Home care in Karlovo municipality" is intended for elderly people who are unable to care for themselves, as well as disabled people who live in the municipality and have a documented need for hourly social or health-social assistance in their regular setting. These individuals will receive home care as well as community-based programs tailored to the basic needs of older people in order to promote independence while minimizing loneliness and isolation. Medical care for people with disabilities will be provided by establishing a model of hourly integrated health and social services. This type of service demonstrates its worth as a means of reaching the most vulnerable segments of the population and ensuring that they receive timely and professional assistance based on their individual needs.
