The present project proposal will improve the quality of life of the elderly and persons with disabilities, by providing hourly mobile integrated health and social services in their homes, aimed at effectively exercising their right to independent living and social inclusion. It is planned to carry out a thorough analysis and assessment of the needs of each candidate user (social and health).On the territory of the municipality of Kirkovo, integrated health and social services will be provided by home nurses and specialized personnel for the provision of health services, and employment will be provided for these persons within 21months. The provision of services will be based on the identified needs and specific needs of the representatives of the target group and the individual social assessment of each one of them. Within the framework of the project, training and supervision of the staff are provided.
